Tested with two servers (potato and RH 6.2) running OpenSSH 2.3.0p1.
Also no output from "ssh -2 host echo test" if using the 2.3.0p1
client (which defaults to -1). No problem with 2.9p2 servers.
I noticed the problem recently because the default protocol version
used by the client has been changed from 1 to 2. It seems there are
bugs in the early protocol 2 implementation found in OpenSSH 2.3.0p1.
It's not really a bug in 2.9p2, but a bug-compatibility problem with
an older version. But 2.3.0p1 is still widely used, so I'd suggest
to change the client to default to protocol 1 (you can always use -2
if you know what you're doing) when connecting to such old servers.

That's a fairly crude approach: ssh has a good deal of code for dealing
with bug-compatibility in more sophisticated ways. I suppose it might be
required since even the client from 2.3.0p1 couldn't get it right,
though. Ugh.
Note that the remote server's version is not known until *after*
fill_default_options() has been called (erasing the distinction between
default options, options you've set in the configuration file, and
options you've set on the command line), so the solution you propose is
not currently possible.
Frankly I would suggest "tell people not to use 2.3.0p1 servers" as an
acceptable workaround. :)
