the system.fvwm2rc95 has the following:

PixmapPath /usr/X11R6/pixmaps:/usr/include/X11/pixmaps/:/usr/local/lib/mini-icons/
IconPath /usr/X11R6/icons:/usr/include/X11/bitmaps/:/usr/local/lib/icons/


the /usr/include/X11 symlink comes with lib6-dev and icons will not
be shown if it isn't installed. The correct way is to use 
/usr/X11R6/include/X11

the same problem is in all examples of fvwm2rc95 that come with fvwm95.
